Welcome!
This information is meant to help guide and direct patients
and their families as they choose a hospice provider.
We recognize that this is an extremely difficult time in anyone’s life.
It is our belief that hospice can provide both the patient
and their family with focused care that will bring comfort,
dignity and peace to all participating in the care of the patient.
Because we understand how overwhelming and challenging this
time can be for the patient and family, we offer free evaluations
to assess the patient for hospice care appropriateness.
At the evaluation we will be happy to discuss the scope and nature
of hospice care with both the patient and family. In addition,
if you have any questions, we can be reached at 765-453-7702.
There is a nurse on call 24 hours a day, 7 days a week to answer your questions.
Life is precious. We believe that our patients deserve the
same loving attention at the end of life that is showered on
babies as they enter this world. Let our compassionate,
